2023.06.06 05:35 SPTCTBP Years of hct above 49, hgb above 16.5 (with some under) but Labcorp has different range? Low platelets. Hematologist not interested because spleen is only "top class in size" & has a cyst but not considered enlarged yet & epo normal, he decided against jak2 test.
40 male, lean, live at sea level, never smoked etc. At 35 at the end of 2016 I did some basic blood work and had a hemoglobin of 17, hematocrit of 49.1, platelets just above range at 152, a few months later I did a testosterone test that came with hct and my test level was at the very low end of normal, hct was 49.9. I was also only 122lbs at this point (later to 140's, now ultimately 160's). Labcorp's range for hgb is 12.6-17.7 but was later changed to 13.0-17.7, hct range is 37.5-51.0 and still is today.
I look into testosterone replacement therapy and do more bloods throughout the year as I am starting to lift, do cardio, change diet, eat more calories etc, platelets start going lower down to 138, I find low vitamin d and b12 and start working on that, and keep lifting. Hgb at 15.8 & hct at 47.7. Still low end of normal test levels but starting to trend up during "noob gain" phase of lifting.
Early 2018 I have 16.1 & 48.6, platelets down to 126. Later 2018 I get bloods through Quest at Dr during std checkup etc right before I got super sick with something that was never diagnosed (neg for strep, all std, epstein barr, flu, tb etc) and my hct was 49.9 on range of 35.0-45.0 & hgb was 17.1 with range cutoff at 15.5 but nothing was said about it. Also had a separate testosterone that was lower than my original in early 2017, it's starting to trend down here.
Home sleep study in 2019 shows ZERO sleep apnea.
Jan 2020 I get a 15.9 & 46.9, platelets down to 128 (cutoff is 150). Nothing else odd but Testosterone has been creeping down, so it's not a driver of erythrocytosis.
Later in 2020 I see a hematologist (seemed out of date, he had been an MD for over 20 years already) after an hct of 51.1, hgb of 17.4 and an rbc of 5.87 (out of range), and platelets at 138. B12 was insane at 1700 He says I likely have immune thrombocytopenia and said "they don't look at hematocrit". Was fighting some weight loss issues here despite lifting and counting calories etc, and no longer having a job where I would walk 27k steps a night was confusing.
He said do more CBC's and I had weird cholesterol on this one so I did another one a couple of weeks later and my hct was 50.1 and hgb 16.1, platelets 132. I get a referral for in lab sleep study (like all of my bloods, this will be out of pocket). End of the year I get a 48.9 and 16.4, but platelets are now at new low of 121. Had a CBC with 48.9 & 16.3, platelets at 127, B12 is through the roof at 1732 when it had previously not responded to shots or supplements. I stopped the supplement, months later b12 still high at 1275. Follow up with hematologist was basically just keep doing cbc often to monitor platelets, no concern with hct or hgb even when elevated the same year and in the past.
Early 2021 I get a 49.2 hct and 16.5 hgb and platelets now down to 110. In lab sleep study in mid 2021 revealed no sleep apnea at all, just zero stage 3 sleep, same as home study from 2019. Testosterone has been trending down, now free test levels are below range and estrogen is too, feeling bad, do two more blood panels before going on TRT in mid 2021. First one shows HCT at 52.1 & HGB at 17, platelets at 116. I do bloods one more time that year (at this point blood work is cheap as hell and I am REALLY trying to avoid going on TRT) and get hct 51.7 and hgb at 16.5, platelets at 116.
I go on TRT expecting hct to go up, it goes to 55 by the end of 2021. I donate blood in early 2022 and right after hct starts going to 59.2, and stays at 59 for 2022 on a few tests, platelets now alternate between just above normal at 160 and low at 140. I donate at end of 2022 and it goes down to 52.5 but climbs right back to 55, then 57.8. Different hematologist's NP says she is sure she felt my spleen and I cringe in pain upon palpation. He comes in and orders complete abdominal ultrasound. I asked with my bloods trending UP over the years, is it possible they would have kept going up anyway if I never went on trt, he went on rant about trt and said we can not know. He was not concerned about PV because epo was normal, I said "TRT drives up epo, we never tested BEFORE trt, so we dont know if it was low before". He said if my spleen is enlarged, he will order jak2. He keeps focusing on current trt blood work showing high counts, but ignores the ones before going on. He's also been an md for years and didnt seem to know the updated WHO criteria.
Spleen comes back as top class in size with a small cyst and a splenule, liver, kidneys, pancreas all look good. He blows it off, doesn't order a follow up and his nurse tells me he doesn't like people doing homeopathic things or taking hormones etc.
Had two weird occular migraines in 2018, I have been itching after a shower for YEARS and it gets better after I donate but still occurs (even if I just wash my feet, my legs light up itching at night), had it before trt. Have a big toe issue that wont heal, started as a foot injury at old job in 2019, it wont heal, 7 months ago it flared and cannot get it under control. MY DPT is baffled as to what to do, MRI shows erosive changes in first mtp joint of big toe, and said to rule out Gout at bottom of report. I started having crazy acid reflux around the time toe flared and am now on meds for it, dr wants me to do upper GI test to rule out ulcers etc. I dont eat garbage that would cause an ulcer, nor take things that would cause an ulcer but get BLOATED TO HELL and back anytime I eat, and this makes it harder to bulk. Fatigue is unreal, without TRT I would be down for the count, I still stay active and lift 4-5 times a week but it's a major struggle, stay in shape (no cardio now with foot problem, used to run hills and climb in the woods) but am run down at work, fall asleep driving etc.
I am not sure if I should just pay for a jak2 out of pocket, and if positive, drive 8 hours to an MPN specialist, or if I could come up with a reason for every single symptom that has occurred over time. The random normal counts in between high counts, which ultimately went higher and higher make me think it's something else.

In drafting this request for help, I think I stumbled myself through figuring it out. I've pretty much re-written it 5 times as I went back and forth testing things and getting closer to the root cause. Since I failed to find my issue previously discussed and solved online, I thought I would still post it to 1) possibly help someone else and 2) see if I'm on the right track. I'll wait to order parts until at least tomorrow....
After a couple years of service since being installed in 2014, our very heavy garage door paired to a LiftMaster model 8550-267 opener instantly seemed to travel at a much slower speed (even with the door detached from the trolley). I had been assuming the motor was about to fail for the last couple years. When I initially noticed the motor had slowed down, I figured out that my door was out of balance (quite heavy for the motor as it would not stay half way open when off the trolley) and I tightened the torsion springs to get the door slightly better balanced as I was concerned something got overloaded and damaged and its days were numbered.
Last week the opener failed (in the up position) with "motor control error code 1-5". With the belt removed, the motor does not have any discernable rotation whatsoever. I can hear a relay close and feel the buzz of the motor for about 300ms before the relay opens and the buzzing stops.
I then disconnected the power and i/o harnesses from the control board, measured 0.9 ohm electrical resistance across 12VDC rated motor and had no issues driving the motor CCW and CW with a jumper connected to the 12V SLA backup battery. I power cycled the unit as well as left it disconnected from power overnight, reinstalled the power harnesses and got the same error code 1-5. Using the manual up and down adjustment arrows on the housing results in the same symptoms.
It seems the 100VA 120:19VAC transformer is used only for driving the motor. I was able to verify 120VAC and 19.5VAC on the primary and secondary windings during that ~300ms while I'm holding the up or down arrow on the controls.
Next,
I measured the output voltage to the motor and measured only 1.80VDC. So it seems to me the AC/DC power supply portion of the control board is the culprit, right? My control board says "45DCBL5 Rev C". [This

I assume it's a safe bet to go ahead and order a new logic board at this point right? (Can you tell I have a pet peeve at throwing parts at problems?).
Any other diagnostic check I should do before dropping the $150 on one? I did find a used one on ebay for about half of that - I'm still thinking through which route I'll go (guaranteed to work per the listing and seems quite easy to swap).
I'm guessing that this is not a common issue based on my previous lack of internet search findings (although I'm sure there are plenty of motors driving heavy non-balanced doors)
I'll certainly be making sure my door is in proper balance before I put the new board/components into service.
Any suggestions or troubleshooting tips would be greatly appreciated!

Draven’s eyes squinted, and he sank deep within himself. Pulling on [Mind Supremacy], Draven blocked out the pain and focused on his inner flame. He pulled on his essence, charging up as much fire as he could. Flames licked the surface of his armor as he built up more flames with [Draconic Pyromancy].
New Mind Supremacy Technique! Pain suppressant
Draven opened his eyes, flames burning deep within them. He cried out as he unleashed the tightly packed fire he contained within himself. Flames exploded out of him, pushing the El’Dresi away from him.
The El’Dresi crossed his arms, blocking his face from the brunt of the flames. The explosion pushed him back, causing his exosuit to leave rough groves along the metal flooring.
“The little disciple has some bite to his bark,” the El’Dresi smiled. He disappeared in a flash of lightning again, using the Flash Step Technique of air dragonborn. The El’Dresi appeared before Draven again, blade zipping towards Draven's heart.
Draven’s heart skipped a beat as he channeled Essence to his Supremacy Talents and entered Ascendancy mode. The Sygaldry along his armor glowed a furry red, empowered by Draven’s Essence. He parried the El’Dresi’s blade and punched the air dragonborn with a fist of concussive fire.
The El’Dresi went sailing into the air, crashing into the ground with a bang.
“Draven, Sync up with me,” Sygvar said. He too had entered Ascendancy mode.
Draven pushed his mind into the Weave. He found the Sympathetic Thread connecting him and his Master. It was thick, a sign of their strong connection. He bound himself to it, allowing himself to become one with his master’s mind, body, and soul.
New Mind, Body, Soul Supremacy Technique! Sync Draven quickly entered the settings of his status and disabled notifications. He thought to leave them on before because he believed they could be helpful. But they were distracting him from the battle.
A wurl wind of air manifested around the El’Dresi as he floated to his feet in Acendecy Mode. The Sygaldry, runes of power, decorating his armor, were a blaze, a green glow emanating from the words of power.
“Now the real fun begins,” the El’Dresi crackled. He Flashed Stepped, disappearing again.
Draven sensed the El’Dresi through the Weave. He lashed out behind him, kicking the El’Dresi in the chest and pushing him towards Sygvar.
Sygvar kicked the legs from under the El’Dresi, causing him to fall.
Draven created a flame missile and fired it point-blank at the El’Dresi.
The El’Dresi conjured a blast of Air, causing the missile to bend off course and detonate into the ground.
He hopped to his feet, sending a chain of lightning at Sygvar.
Sygvar blocked it with his sword. He then heated his blade and cut into the armor of the El’Dresi.
The El’Dresi pulled back in pain.
Draven came from behind, delivering a cut to the El’Dresi’s leg. Causing the air dragon born to fall to his knee.
“You guys are good, too good. I guess I need a little help,” the El’Dresi said as he pressed a button.
“You talk too much,” Draven said, smacking the El’Dresi.
Gates squeaked open, and the ground began to rumble. Hollows came pouring into the room. Marrows ran on all four, with Spikes trailing just behind them. Bone Devils mixed in with the horde, pushing aside the weaker hollows to get to Draven and Sygvar.
The El’Dresi jumped to his feet and sprinted toward the incoming horde of hollows. Draven and his master raced after him before he could get to safety.
Draven unleashes a cone of fire. Superheating the air as it impacted the El’Dresi.
The El’Dresi conjured a shield of air, deflecting the blast of flames. And letting the momentum push him up and over the horde of hollows. He disappeared in the wave of bodies, crackling like a madman.
“Damn it,” Sygvar hissed. “Draven, stay close to me.”
Draven drew near his master. Only a few feet away from him.
“We’re going to clear a path towards the computer terminal. I want you to have Ari download the files off of it.” Sygvar yelled over the horde of hollows. His sword was ablaze with fire as he sliced throw a Marrow.
“The base will self-destruct in five minutes,” a voice boomed from speakers in the room.
“That bastard! Draven, don’t hold back! When need to get to that console!” Sygvar’s voice warped and deepened as his body transformed. His exosuit became more organic, melding to his body like a second skin. Wings and a tail sprouted from his back. His fingers and toes sharped into claws, and flames manifested on his arms and as a crown of fire above his head. Sygvar flew into the mob of feral mutants, becoming a living storm of fire and death.
Draven underwent a less impressive transformation. The red glow from the Sygaldry and flames manifested as jets along his arms and legs. The Hollows before him slowed as he sped up his perception time.
Plums of fire rocketed Draven into the horde of Hollows, following his master. His sword glowed as he sliced throw the feral mutants. The Marrows and Spikes were cut down like butter, but the Bone Devils were still challenging. But they were cut down in a few strikes while Draven was in his enhanced state.
The duo gradually cut their way toward the terminal. That’s when Draven got a spike of danger from the Weave. He was too slow and was slapped across the face by a giant claw. Draven backflipped, distancing himself from the new foe.
This creature was different than the others. This was a Wargoth, a mutant with the power of bone and muscle. Four muscular tentacles protruded from the creature's back. Segmented bone covered the spasming muscles as its tentacles wiped around. It roared, spitting saliva all over the ground. Then charged Draven, its four claws shaving the metal flooring.
Draven kicked the creature in the face, sending out a current of fire.
The Wargoth went sailing towards Sygvar. It lashed out its tentacles at the Master Al’Manti.
Sygvar’s blade ignited in flames, and it blurred, severing the Wargoth’s tentacles.
Draven came up from behind, stepping on telekinetic steps. He then rocketed downwards onto the Wargoth with a jet of flames. He concentrated, his flames burning hotter and hotter. Eventually ultimately shifting from their reddish yellow hue to a bright blue. His blade cracked the bone exoskeleton around the Wargoth’s skull, bathing the creature’s brain in blue flames.
The creature thrashed about in pain, then went still.
“Draven, go get the data!”
“On it,” Draven said as he sprinted to the console. He took out a cable from the back of the neck of his exosuit and plugged it into the console. “Ari download all the data from the storage.”
File after file zipped past Draven’s hud for his exosuit. The download only took a second to finish.
“I’m done, but the data must be decrypted,” Ari said.
“That’s fine,” Draven then unplug the cable leading from his suit to the console. He then ran over to his master, “All done.”
“Good,” Sygvar picked Draven up and then took to the sky with his wings. Above the increasing number of Hollows, Sygvar sped towards the exit. The hollows raged as they chased after the air-born duo. Quickly they exited the base, landing on Tywen’s back.
“Take to the sky, quickly!” Sygvar yelled as he set Draven down.
Tywen beat his wings, and they rapidly ascended. Soon after they left the ground, hollows poured out of the El’Dresi base. They swarmed the area they had just been. Then an explosion destroyed the hill the base was built into and the flora around it.
Draven opened his status and instantly dumped nine path points into his path.
